What is an Advance Medical Directive
An Advance Medical Directive (‘AMD’) is a document you sign to instruct the doctor treating you (where you are terminally ill and unconscious or incapable of making a rational decision) that you do not want any artificial means of prolonging your life, such as the use of life-sustaining equipment (eg artificial respirator), where there is clearly no hope of finding a cure on either a temporary or permanent basis.

Advance Medical Directive
An Advance Medical Directive (AMD) is a legal document you sign in advance to inform your doctor that you do not want the use of any life-sustaining treatment to be used to prolong your life in the event you become terminally ill and unconscious and where death is imminent.
The AMD can be made by any person, aged 21 years and above, and of sound mind. The AMD form is a legal document which must be completed and signed in the presence of two witnesses before it is returned to the Registrar of AMDs. The patient's doctor must be one of the two witnesses, while the other witness must be at least 21 years old. In addition, both witnesses must not have any vested interests in the patient's death.

this is a major decision.. wish that it's not a this or that thingy.... would be more ideal if can choose max duration to be put on artificial means of prolonging life. it's certainly not a good job to be the one who pulls the plug... |

Its a decision difficult to make.
Coz all these involved alot of factors.... thinking of our family etc....etc...
Years ago when my grandpa was ill in hospital, doc did ask us whether to pull the plug .... we will so indecisive, coz we know.,
we would want our grandpa to live but on the other hand... we dont want to see him suffer...

I think it is good to have something like this to make your own decision with. Rather than asking a difficult question to the family who will definately be in a moral dilemma sometimes maybe the $$ factor etc. It's hard to say this, but at times, prolonging at a lost cause is nothing but a burden to those alive, mentally and financially. For me I would sign it. |

I wonder if I can state on the AMD form that I want it to take effect only after I'm 62 years old (retirement age)? After all, what's more to do and see after 62 years old? Skin has wrinkled and joints are brittle... :p |

well, if one is afraid that his/her organs are removed after death, u can opt out of HOTA by filling up a form. |
